• Health Insurance

Your PPI is collected when you register for an appointment on the Site, subscribe to an e-newsletter, opt-in to receive a text and email messages, provide patient testimonials, and sign-up or sign-in to the “Patient Portal” on our Site. The use of the Patient Portal requires you to create a username and password. We and our service providers, Patient Fusion and LabCorp, follow all HIPPA compliance guidelines to ensure your PPI is protected.

POLICY FOR INTERNATIONAL VISITORS

This Site is providing information and services for individuals residing in the United States. If you are a Site visitor from outside of the United States, please be advised that you are sending information to the United States where our servers are located. We do not recommend that you share personally identifiable information with us because the United States may not have data protection laws as comprehensive as your country of residence, such as GDPR for members of the European Union. If we discover that we have information from a resident outside of the United States, who is not a current patient of Mason Park Medical Clinic, we will delete the record immediately.

POLICY REGARDING MINORS

This Site is intended for individuals over the age of 18. Anyone under the age of 18 should only use this Site with a parent or guardian. We do not collect or maintain information from anyone known to be under the age of 18, and no part of this Site is designed to attract anyone under the age of 18. If we discover or are notified that we have received any information from a minor in violation of this policy, we will delete the information immediately.

POLICY ON DATA RETENTION

We only keep PPI collected from Patients for as long as they remain with Mason Park Medical Clinic. Users’ accounts stay active for a limited period of time as long as we need it to fulfill the purposes for which we have initially collected it unless otherwise required by law. We will retain and use information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements for a period of 5 years for accounting and billing information and a period of 10 years for legal information.

INDEMNITY

By using this Site, you agree that we are not responsible for: (i) any disclosure of your personal information made by you to a third party through your use of this Site; (ii) any disclosure of your personal information obtained illegally from us; or (iii) any accidental disclosure of your personal information made by us.
