What are your hours of operation?
Does your clinic offer after hours care?
What information is needed to schedule an appointment?
- Patient’s full name (including middle initial)
- Date of birth
- Current address and phone number
- Current insurance information
- Purpose of the appointment*
What is the policy for missed appointments and cancellations?
To respect the time of our patients and providers, we kindly request that you notify us within 24 hours if you need to cancel or reschedule your appointment. Please be aware:
- A fee of $35 is charged for appointments that are not attended or canceled with less than a day’s notice.
- If you arrive more than 15 minutes late, your appointment may need to be rescheduled, and a $35 fee will apply.
Are there any fees for office services?
Yes, we have nominal fees for specific services to cover the administrative costs:
- No Show/Same Day Cancellation: $35
- FMLA Documents: Starting at $50
- Printed Lab Results: $5 (There is no charge if the results are sent via fax or email.)
- Work related forms, Letters and Disability Forms: Starting at $25
- Medical Records: $25 for the first 1-20 pages; then $0.50 per page. (For patients, there is no charge for records sent via fax or email.)
How long does it take to process a refill request?
Please allow 48 to 72 hours for us to process your prescription refill requests. This time frame ensures we can review your medical records and make the best decision for your health.
When can I expect to receive my test results?
Please note that your lab results will be available to view in MyChart. Normal results will be communicated through MyChart; however we strive to communicate abnormal test results within 3 to 5 business days. Our team will reach out to you directly with the results and any next steps and recommendations by your provider.
How long does it take to get a referral or prior authorization (PA)?
The process for obtaining referrals or prior authorizations can take 5 to 7 business days. We appreciate your patience as we coordinate with other healthcare providers and insurance companies to fulfill these requests.
What is the turnaround time for completing forms and documents?
Please allow us 3 to 5 business days to complete any forms or documents you’ve submitted. This includes FMLA documents, disability forms, and other medical paperwork.
Do you offer onsite lab services?
We have onsite lab services for the convenience of our patients. Lab services are offered by LabCorp.
